Description of Services Our Services are designed to assist members with two primary functions: Doctor Finder: We help you find healthcare providers (e.g., doctors, specialists) who are reportedly in-network with your stated health insurance plan. Bill Negotiation: We assist you in communicating with healthcare providers or billing departments to negotiate a medical bill you received. IMPORTANT SERVICE DISCLAIMERS We Are Not a Medical Provider. We do not provide medical advice, diagnoses, or. Our service is informational only. Always consult a qualified medical professional for health concerns. If you are experiencing a medical emergency, call immediately. We Are Not an Insurance Company. We do not provide health insurance, determine eligibility, or make coverage or decisions. We are an independent service. We Are Not a Financial Advisor. We do not provide financial or legal advice regarding medical debt. 2. SMS & Electronic Communications Your use of our Services is conducted primarily via SMS (text messaging). Express Consent: By sending an initial SMS message to our designated phone number, you are initiating a business relationship and expressly consent to receive electronic communications from us, including automated and non-automated SMS messages, at the phone number you used. Purpose of Communication: You agree that we may send you messages related to your service requests (e.g., doctor matches, questions about your bill, status updates on negotiations) and for administrative purposes. Opt-Out: You may opt out of our SMS communications at any time by replying STOP to any message you receive from us After you send the STOP message, we may send you one final confirmation messageHelp: For assistance, reply HELP to any message. Costs: Message and data rates may apply to all SMS messages sent and received. Check with your mobile carrier for details. 3. Your Responsibilities as a Member To use our Services, you agree to the following: Accurate Information: You will provide true, accurate, and complete information. For Doctor Finder, this includes your insurance plan details, location, and the specialty you need. For Bill Negoti, this includes accurate details about your bill, the provider, your insurance, and the services rendered. Verification of "In-Network": We make a best effort to identify "in-network" providers based on the information you provide However, provider networks and affiliations change frequently. You are solely responsible for independently verifying a provider's in-network status with your insurance company before receiving services. We are not liable for any out-of-network charges or other fees you may incur. Authorization for Bill Negotiation: To use our Bill Negotiation service, you may be required to provide us with a written authorization (in a form we provide) that allows us to access your Health Information (PHI) and communicate with your providers and insurance on your behalf. You agree to cooperate and provide any necessary documentation. 4. Service-Specific Disclaimers Doctor Finder Disclaimer: We do not guarantee that we will find a suitable available provider that matches your criteria. We do not endorse, recommend or guarantee the quality of any provider we suggest. Bill Negotiation Disclaimer: We do not guarantee any specific outcome for our Bill Negotiation service. We do not guarantee that your bill will be reduced by any amount, that we will be successful in negotiating, or that a provider will agree to any change. Our service is limited to facilitating communication and negotiation on your behalf. 5. Privacy Your privacy is important to us. Our data practices, including how we collect, use, and share your personal information, are described in our Privacy Policy. Please review it carefully. Termination We reserve the right to suspend or terminate your access to our Services at any time, for any reason, without notice, including for any violation of these Terms. You may terminate your relationship with us at any time by ceasing to use the Services and replying STOP to our SMS messages. 9. Governing Law & Dispute Resolution These Terms shall be governed by the laws of the State of [Oklahoma], without regard to its conflict of law provisions.
